It is called the Ukraine Defender of the Motherland medal.

I found this description:

Gilt metal medal with eyelet for ribbon suspension; the face with a scalloped escutcheon imposed on a sword and bearing a radiant map of the Ukraine inscribed ‘Уκραϊна’ (Ukraine), oak and laurel leaves below right, circumscribed above left ‘ЗА ХИСНИКУ ВІТЧИЗНИ’ (for Defender of the Motherland); the reverse with a sprig of laurel above the Ukrainian tryzub (trident); on original ribbon and pentagonal hanger mounted for wear. The medal was instituted by Decree of the President of the Ukraine 1299/99 dated 8 October 1999 and is intended to recognize veterans of war status who live in the Ukraine and the citizens of other countries who participated in liberation of the Ukraine from German occupation and other Ukrainian citizens who display personal bravery in defending the interests of the Ukraine and contribute to its defense.
