Hi Ryan,

 

Right side, slogan: 祈武運長久 Ki Buun Choukyuu - Prayers for continued luck in the fortunes of war

Right side, name:  Last name: 谷口 Taniguchi, First name: 源吾 Gengo 君 Kun – Mister

 

I haven't attempted the left side but the writing looks too faded and difficult to translate from the photo.
